Vital Records

The Pierce County Register of Deeds Office issues certified copies of Birth, Death, Marriage, Divorce and Domestic Partnership records as follows:

Birth - Wisconsin Birth Certificates may be obtained from any Register of Deeds office in the State of Wisconsin.

Death - Deaths that occurred in Pierce County prior to September 1, 2013 must be obtained from our office. After September 1, 2013 may be obtained from any Register of Deeds office in the State of Wisconsin.

Marriage - Marriages that occurred in Pierce County prior to June 21, 2015 must be obtained from our office. After June 21, 2015 may be obtained from any Register of Deeds office in the State of Wisconsin.

Divorce - Divorces that occurred in Pierce County prior to January 1, 2016 must be obtained from the Pierce County Clerk of Court Office. After January 1, 2016 may be obtained from any Register of Deeds office in the State of Wisconsin.

 

Ordering by Mail

1. Print the appropriate vital record application form:

Birth Certificate Application
Death Certificate Application
Marriage Certificate Application
Divorce Certificate Application
Domestic Partnership Application
Domestic Partnership Termination

2. Complete all parts of the application form.

3. Include payment of $20 for the first copy and $3 for each additional copy of the same record. Make check or money order payable to "Register of Deeds".

4. Include copy of valid Driver's License or other identification as listed on the back of application form.

5. Include self-addressed, stamped return envelope.

6. Mail completed application, payment proper identification and return envelope to:

An Apostille is a specific authenticating certificate for a birth certificate that may be required by other countries. In Wisconsin, an apostille can be issued by either the office of the Wisconsin Secretary of State or the WI Dept of Financial Institutions.

Correct a Birth Record

State of Wisconsin Vital Records 608-266-1371 will assist with corrections if under 12 months old and will provide an Amendment Packet for birth record that is over 12 months old.
