The goals of the Department's Home Care Program are to provide quality services, preserve the health and safety of County residents and promote client independence with their health care needs; all in the comfort of their own homes. The nature of practice is general, yet comprehensive health care is provided to citizens of all ages.
If upon referral, we have resources to meet all the clients needs, one of our Registered Nurses will perform an initial evaluation to determine if they qualify for home care. The client must show a reasonable expectation that their medical, nursing, and social needs can be met adequately in their home setting. They must have adequate physical facilities with or without modifications in order to allow for safe in-home care. Generally cares are provided 1-3 times per week, but based on client needs and physician orders, cares can range from bi-monthly to daily. Type, frequency and duration of cares are always individualized for each client.
