The goals of the Department's Home Care Program are to provide quality services, preserve the health and safety of County residents and promote client independence with their health care needs; all in the comfort of their own homes. The nature of practice is general, yet comprehensive health care is provided to citizens of all ages.
Skilled Nursing, Home Health Aide and Personal Care services are provided through the Department directly.
Physical and Occupational Therapy services are provided to the Department under contract.
All clients must be under the care of a licensed physician. All home care services are only provided with orders approved and signed by their physician.
If upon referral, we have resources to meet all the clients needs, one of our Registered Nurses will perform an initial evaluation to determine if they qualify for home care. The client must show a reasonable expectation that their medical, nursing, and social needs can be met adequately in their home setting. They must have adequate physical facilities with or without modifications in order to allow for safe in-home care. Generally cares are provided 1-3 times per week, but based on clients' needs and physician orders; cares can range from bi-monthly to daily. Type, frequency and duration of cares are always individualized for each client.
Services Include:
Registered Nurses
· Skilled Assessment
· Administering medications and treatments ordered by your physician
· Comprehensive health education
· Blood work drawn in home, IV therapy, care of wounds
· Supervision and case management of all care provided in your home
· Foot care in your home
Licensed Practical Nurses
· Assisting registered nurses in your total care
Dietary Consultant
· Diabetic and other special diet assessment and instruction
Home Health Aides
· Providing bathing and grooming needs
· Assists with rehabilitative exercises under the direction of your nurse or therapist
Physical Therapy and Occupational Therapy
· Assisting and teaching you to use your muscles, limbs and communication skills to reach your full functional level in your home
· Home adaptive equipment and professional evaluations

are eligible to participate in this service which provides: Assessment of circulation abd skin integrity A warm water soak Nail trim lotion massage We do provide In-Home Foot
Care by appointment only. The cost is $50.00 to be paid at time of service. Please

that electronic communications with the department are not encrypted and
confidentiality cannot be assured. Pierce County Public Health Staff
will not discuss your private information through email until we have
received a signed Agreement on the Use of Electronic Mail for Client
