Long Term Support
The goal of the Long Term Support Unit is to help disabled and/or elderly people remain in their own homes.
Staff assists people in achieving this goal through a needs assessment process which leads to the development of an individualized plan of support.
Long Term Support care managers support clients' desire to stay in their own homes to continue to be part of the community and to be close to family and friends.

Planning and on-going care management.Ø
Supportive home care to help you deal with your day to day living situation.Ø
Medical transportation.Ø
Daily living skill or vocational training for the developmentally disabled.Ø
Home modifications and adaptive equipment, related to your disability, to make living on yourown more manageable.
Ø
Guardianship/protective services for people unable to make decisions on their own.Ø
Respite services provide relief to caregivers of disabled family members.Ø
Alternative living arrangements are coordinated for people who may be unable to live independently.Programs
Eligibility for the various programs depends on an income and asset evaluation and the extent of your
need for services.
Community Options Program
Designed for elderly or disabled persons who have a long-term or irreversible illness or disability. The goal is to help people remain in their own homes.
Community Integration Programs
A variety of programs for the elderly and the physically or developmentally disabled. These programs are similar to Community Options in nature and intent, but eligibility standards differ.
Family Support Program
Designed to help families with children who have severe disabilities.
Other Programs
Special funds are available through the Office on Aging and general state and county Community Aids programs for services such as supportive home care, respite care and chore services.
