Basic Information on Applying for a Passport
You may obtain passport applications from our office or this website: http://travel.state.gov
You must submit a certified copy of your government issued birth certificate. Hospital documents are not accepted. Your birth certificate will be returned to you in a separate mailing from your passport.
You may arrange for the County Clerk's Office to take your passport photos or obtain them elsewhere. You will need to contact them at 715-273-6744 to arrange your photo time. The cost is $17.50 for the two photos.
We will need to make a photocopy of your driver’s license or state ID card. If it does not have your correct address (or only has a PO Box) you must also provide a document showing your correct residential address.

(1) This payment goes to the U.S. State Dept. and must be by check or money order as we can not send cash and do not take credit cards. For multiple applications submitted at the same time: Each one must have its own check for the U.S. State Dept.; these fees can not be combined.
(2) The $25 per application execution fee paid to our office (Pierce County Clerk) may be combined for multiple family members and may be cash, check, or money order.
Allow 6 weeks for normal processing.
FEES FOR EXPEDITED PROCESSING: Additional $ 60 for each application using regular mail service (approximately 3-4 weeks processing time). To ensure receipt of the passport within 2-3 weeks, the expedited fee would be $ 74.96 which includes $14.96 for Express Mail postage.
When Completing The Application:
Ø Use permanent blue or black ink. PRINT NEATLY using ALL CAPITAL letters.
Ø In all date spaces: there should be 1 digit in each box with no dashes or empty spaces. Example: if your date of birth is May 8, 1963 you would enter 05 08 1963, NOT 5-8-63.
Ø Use an “X” in check boxes, not ü.
Ø No white-out or erasures are allowed. If you make an error, try to correct your mistake as best you can or complete a new form.
Ø We must witness your signature so do NOT sign the application until told to do so.
